  3. Return:  your form – along with any supplemental records, lab results or other documentation you wish to have on file – to [email protected] at least ONE WEEK prior to your intake session.

*This form is a critical component to making the most of our precious time together. Please take the time to fill it out completely. If an intake form is not on file at the time of your initial appointment, the appointment will be cancelled + rescheduled at the next available time slot.
